You must be very familiar with Amember - if not, please do not apply.

1. I have existing website and am using Amember [url removed, login to view]

2. Please see attached PDF - this page is currently html. you do not need to create the html

3. Need this page to use data entered in white rectangles to create an amember profile/membership

4. regarding field- Open HousePromo Code:

they must use the correct promo code - if wrong code, page should display message

5. once they press submit, it takes them to the protected folder (the amember product is already set up.
