Complete PHP Mail script with upload image as attachment

Hi, I want a complete working script, that has the html form and the php script that handles the sending (it would probably have MIME). The Html form should have the following: (Name Field, Phone number, City, email, Comments textarea, and the most important thing Browse button to upload a picture (Gif, jpeg, bmp), and this pic should be included in the email as an attachment. I want the complete script to be workable. Before I pay you, I will download and install the script on my server, and if doesn't work smoothly without any configration (except for changing from, to, subject), I won't pay anything. This should be a really simple project, or you might already have the script ready, I just don't have the time to look for a script on the net. My budget for this project is between $5-$15 Thanks.
