Connect existing PHP to existing database

I have a very simple (~ 150 lines) php page (script) that waits for a query and then matches 3 ID's together and returns those ID's a grouped match. It was built by another freelancer and works great except that he put it on a free hosting site and now it's reached its data limit so it stopped working. I have a godaddy hosting plan and want to migrate this php page (I have the source) to godaddy but the freelancer who designed this project is not around so I am looking for someone to do this job quickly. It's a very simple setup, but I get the following error when I try to connect. [login to view URL]
