Create a data entry page using PHP/MySQL/AJAX

I need to create a data entry form which has to? be wriiten in PHP/MySQL. The form has approxiamately 25 fields and you would need to the folllowing. 1. Create a class which reflects all these fields as properties. 2. Method to add/delete/update. 3. Field validation.(Ajax) 4. Design the form with CSS Please bid.
