Create 'friendly' unique field within CSV file

Script required to take salutation, initials, firstname, surname and other fields from CSV file and populate a new field with a unique combination of two or more of these fields. For example, assuming that the name Mr Bob Smith occurred on 6 occasions in the CSV file the unique combinations may be.... Record 1: Mr, Bob, Smith? -> BobSmith Record 2: Mr, Bob, Smith? -> MrSmith Record 3: Mr, Bob, Smith? ->? MrBobSmith Record 4: Mr, Bob, Smith? ->? BSmith Record 5: Mr, Bob, Smith? -> MrBSmith Record 6: Mr, Bob, Smith? -> SmithBob
