Corrected: merchant order form perl module modification

Modify merchant order form perl module. 1. Improve comments so new users can understand what arrays and variables need to be changed for their application. 2. Modify merchant order form module to apply 5% discount to total $ value of line items where more than 5 of a particular item are ordered. There are 300+ line items, about 50 items per html page. Multiple products can be ordered on one page. Customer puts in the quantities of each item for mutliple items then adds to cart, goes to next catalog page and adds more product.
