Create Great Looking GUI

Create a great looking GUI that will do login with username // password to my php database. Will lock each username with a cpu id or hardware id to prevent sharing. GUI will send information to php database to verify before allow to use. The gui will have a scheduler when to run certain function that is to be chosen by users. Please bid only if you can provide some great looking samples // project that you previously created. I would like you to attach a picture of the best looking GUI you made. Don't bid if you cannot provide sample of your previous work. Dont bid if you cannot be communicate easily. I had someone accepting the project and dissapear after. Not very cool.
