App IOS / Android + Web Application Development

I need an app / web applications developper ..

I have couple quizzes, plain html / css / php , where the user enters some info, answers couple yes / no questions , and either enters his email address and gets the quiz results delivered to his inbox, or simply clicks submit and gets to see the results directly ..

I have both options for different purposes ...

Now, I need someone who can turn a plain html / css / php into something like this

more modern, slick, responsive ...

A simple box in the middle of the page that contains the quiz .. Questions seperated / steps etc .. kust like the example I gave ... (maximum 12 steps)

That will be in the online version ..

I also want an application developped for the same purposes ..

Both on Android and IOS , compatible for smartphones and tablets

The application will be simple , logo on the top left, top right is an upgrade button for the premium version ..

Then under that a search box, and under it "Latest Quizzes" and a list of the latest quizzes we add to the application.

So basically I want my quizzes in this application , and the ability to add more as we go ..

each quiz must have tags, so the search box can rely on the quiz title + tags ..

The premium version, comes with no ads, and in the quizzes where ir's required to enter the email address to receive the results, we will give the user the bonus to get the results displayed directly without having to enter an email address ...

Note that I don't need it developped with Titanium .. Sencha touch is preferred .. worse case scenario phonegap

The code of the quiz (php) along with email integration is ready .. and I have someone who does develop those quizzes for me,

But it will be a plus if you can do that as well, so you secure repeat orders from me to develop more quizzes as we go ..

Your proposal is very important, and I read them carefully, so please don't just through in "I can do it" and move on ..

Proposal amount is manatory ... I repeat mandatory .. You can leave the official amount empty, but in your proposal mention an estimate for your services ..
