Broken Email Form

Until earlier this month our auto-emailer was working. However due to the large volume of virus our hosting provider has changed the system, and we now get an error when an email form is trying to be sent. Below is the URL of the faulty webpage. The default URL for registration currently has an offline message, until the above problem is corrected..
