Acrobat Javascript Function Button

Looking for experienced Javascripter. Must be able to create function/script so that when the submit button is pressed, the javascript function in the attached file is called. It needs to pull a text box (i.e. text1) and perform the function with a few documented modifiers (boolean, etc). See attached for more info. This project can be done in a single line of code. This is an emergency project, coder must be available IMMEDIATELY to begin work.
