Create a FACEBOOK Widget

Hello, I am looking for a facebook application widget developer that can design and develop for me a facebook widget. What I want to do is create a widget where a person can answer a few questions and depending on their answers, they get labeled something... This is like one of those quizzes that people take all the time titled... "What type of car are you" or "what celebrity are you most like"? Etc, etc... Well I want to create one of those quizzes... I want the user to answer a few questions and then based on their answers they are either A, B, C, or D... and then they get to embed that image onto their facebook profile... Which would then link to the quiz, etc, etc.... Looking forward to hearing from you and any ideas you may have.
