The proces behind every form is basically:
* users fills in form. some fields are mandatory. some fields have validation, some fields use a calendar and so.
* user receives a confirmation e-mail in which field information is used.
* a pre-defined e-mail address receives an e-mail in which field information is used
* all information is stored (for example in MySQL or a file)
My suggestion is to use a 'plugin'
