Actionscript 3 and Flash Troubleshooting

This should be a simple project for anyone who knows PHP and Actionscript 3. I have: A functioning Flash AS3 website (including original FLA file) that needs some final touches. Site here: [login to view URL]~akimball/portraits/ I need: 1. a flash contact form (preferably functioning by calling a php file) 2. a flash form that directs users to a url (with user input appended to the url) 3. An event listener that moves a movieclip between two frames based on a slideshowpro component 4. Troubleshooting: the UIscrollbar component does not display correctly when the SWF is in a browser Timeframe: Delivery within 3 days of choosing provider. No deposits. I will do full escrow.
