We're looking for some *expert* assistance in building a moderately sophisticated series of forms (~20 questions) utilizing drag and drop selection and ordering of values for multiple questions. One of the tricky bits is form persistence, when the user moves from one page to another the values need to be saved and when a member returns to a given page, all values in drag and drop, radio buttons and drop downs should be auto-populated to reflect previous values selected. Users will be able to add new values that will be saved to the DB for future users, then the Drag and Drop refreshed so that that option can be picked. The system also needs to use vBulletin authentication/accounts, but we can provide some assistance with that. The final slightly complex part is the ability for the user to specify up to 3 "concerns" and be shown (via AJAX) resolutions matching those values. The user will be able to add their own concern and either choose an existing resolution or define their own. There are other parts of the forms that will use AJAX to pull the top three answers etc… This is part one of a series of these forms and is intended to be the prototype for the others, so clear, well documented code is a priority. We have html prototypes (using checkboxes instead of the desired drag and drop) that can be easily converted to PhP as well as a database design to start from. We also have expert database assistance available that can iron out any problems with schema structure and/or query design. If you think you have the skills to tackle and complete this project in a reasonable amount of time, we can provide the prototypes and initial DB layout so you can develop a clear bid. We have a group of future projects that we would like assistance in developing for our site and we're looking to build a long term relationship with a developer that can help with the difficult bits. Thank you in advance for your time and consideration.

Platform is PhP 5.1.6/MySQL 5.0.27-standard-log running on Apache [url removed, login to view] on a CentOS VPS box with *very* hefty capabilities. We have a full testbed environment available for development/testing. We’re primarily looking for PhP/MySQL with AJAX/jQuery framework as the main development tools but are willing to be convinced otherwise.
