A banking web site wants to convert seven (7) application forms to HTML. The forms cover applications for opening accounts with the bank for personal, business and merchant customers, as well as three applications for credit card customers. The contents of the forms are to be saved in a MySQL database. The applications are fairly lengthy, and will likely span a number of pages each. Some validation will be required but the exact extent of it is not yet known; simply a number of fields will be required. The functionality will also include the ability to output a PDF of the form, bearing some resemblance to the paper-based forms. The chief intent is to allow the customer to provide a signature where applicable. These forms are attached for reference. The deliverables will include the complete source code for this project and database schema. The deadline for this project is aggressive, so I will prefer bids that can provide guarantees of fast delivery.
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows? (depending on the nature? of the deliverables):
a)? For web sites or? other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software? installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
## Platform
The final product will be running on a PHP/MySQL platform, and the results should be compatible across browsers -- IE, Firefox, Safari.
