ASP Send Email with Attachment with fields

I need an asp form doing as quick as possible.
I have attached the form.
Basically I want to send an email out from an asp form with an attachment in a particular format.
File [url removed, login to view] has got the format of the email listed and does send an email out with out any problems but I have problems attaching an attachment to the same form.
File [url removed, login to view] explains how this can be done but I just cannot work out how to combine the two together.
Can you think of a way of doing this.
If combining the two is not possible I need an asp form doing asap…. Which will have the following format of the [url removed, login to view] but sends an email out to an address from the drop down
The address from which the email goes from is always changing depending on what email address is selected from the drop down list…
The address to which the email goes to is always static.
With a subject of CHANGE#NEW
And a body of the email should be in the format below….
Message = Message & "<DATA><NAME>" & strName & "</NAME><DESCRIPTION>" & strDesc & "</DESCRIPTION><BENEFITS>" & strBenefits & "</BENEFITS><PRIORITY>" & strPriority & "</PRIORITY><INSTRUCTIONS>" & strInstructions & "</INSTRUCTIONS><REQUIREDBY>" & strRequiredBy & "</REQUIREDBY></DATA>"

This detailed will be entered in an asp form as follows
## Deliverables
1) Complete and fully-functional working program(s) in executable form as well as complete source code of all work done.
2) Deliverables must be in ready-to-run condition, as follows (depending on the nature of the deliverables):
a) For web sites or other server-side deliverables intended to only ever exist in one place in the Buyer's environment--Deliverables must be installed by the Seller in ready-to-run condition in the Buyer's environment.
b) For all others including desktop software or software the buyer intends to distribute: A software installation package that will install the software in ready-to-run condition on the platform(s) specified in this bid request.
3) All deliverables will be considered "work made for hire" under U.S. Copyright law. Buyer will receive exclusive and complete copyrights to all work purchased. (No GPL, GNU, 3rd party components, etc. unless all copyright ramifications are explained AND AGREED TO by the buyer on the site per the coder's Seller Legal Agreement).
