Customer Information Package - Redesign, pull data from a Database

Looking for someone to take existing Sales document, rework into a newer sleek design, have it pull information from from an database or (XML? Access?) document for Customer Name, Field Name, Year, A field for Special comments paragraph that could be left blank if needed, Contact Names/telephone/email for multiple people in a 3x? table that shows all contact reps.
Looking for something that will pull from the database and export the document as a PDF that we can send to a printer. (Currently a lot of our Customer information Packages are going out with improper information because it is buried in text.)
