Copy design of signup form using CSS, Javascript

I need to copy the signup form design from this page: [login to view URL]

There will be a few small additions. Namely:

1) There will be two fields with checkboxes. If user selecs field A, the form should show 3 more fields.

2) Some checkbox fields with have associated prices with them (say $100 each). At the bottom of the form user should see total amount selected (ie if user selects two fields, total shown should be $200)

These are all very simple things to do if you know CSS. I think this task should not take more 1 hour of work and 1 hour to debug my changes, so please don't bid above $30. I would also expect this to be done in 24 hours.

Please see my feedback - I am a very fair employer, provide exact specifications and pay immediately. However, please bear in mind that I know this stuff myself, just don't have the time to do everything. In other words - I don't like to fool around.
