Acrobat forms converted to .html or text/php file

Hi, im looking for a programmer that can convert filled in (by clients) acrobat reader files to PHP or Text files in a certain format. For example if a client fills in a acrobat reader form like: What is your name [John Doe] It has to show up in a text file like: name = john doe that's basically it :-), i believe it's javascript programming.
