Build a Form for a Website

I need a webpage where clients enter information and submit it to me. This needs to be done as a Web Input Form like "Contact Us", but it is going to be much more advanced. The design will include: Check Boxes, Radio Buttons, Slider Inputs and many other components.

All the elements are “pre-coded” so, it is a matter of putting things together.
You would need to be very skilled in HTML & CSS, etc.
Only applications starting with "Coding" will be read. To be considered, you need to go to the above link (Smart Forms) and let me know - how easy it would be for you to create the form (using coding only). Also, please let me know your estimate of hours you would require to complete this job.
To give you an Idea of the extent of this work I have attached the Form that needs to be modified (it was created in Adobe LiveCycle Designer). I am entering my Budget as $50, but this can be negotiated (Up or Down), subject to your skills, future potential with us, etc.
Also, my Project End Estimate will depend on the date we start it.
