Fix two AMP errors on my website

I have recently created an AMP website, and there are still two errors on my site that I need fixed. The site works fine on a desktop, but does not work perfect on a smartphone or tablet. You will have to look at this site on a smartphone/tablet to see the errors.

The ‘Free Analysis’ form does not show up. The error needs to be fixed on this page;

Also, the ‘View non-AMP Version’ button at the bottom of each page doesn’t work. When you click on this it should take you to the full non-AMP site.
If you already know about AMP then you may be a perfect candidate for this job. If you don't already know about AMP then you should. This may be a perfect time for you to learn.
I will only pay for this job if you can fix the two errors listed above. I will not pay if you cannot fix those two errors.
I have attached two screenshots. One is the form that should show up on the 'Free analysis' page. The other is the link at the bottom of every page on my site that says ‘View non-AMP Version’ that should take a user to the full site.
