Creative illustrator/writer for a Workbook

I have a book about health. I need a passionate and experienced writer and illustrator/designer to create a workbook based on my book. The workbook would include some new and original text, lots of tables, some illustrations, bullet lists, and self help tools. At least 130 pages needed . I also need to see your previously created workbooks before I hire. Potential for more workbooks later. Quality workbook needed to be designed, from an existing book. A chapter of the book is attached. There may be around 15-20 or maybe a little more workable chapters like this in the book. The complete book will be delivered to the winning person only. The style of the workbook should be classy and elegant, black and white but as a model please see the attached workbook sample. The sample is in a totally different field. The workbook needs to have a little bit of text, and lots of graphs, infographics, tables, tracking systems, tips, bullet point lists, cheat sheets, etc. The winner shall be able to read the book and distill the essential information and put it in tables, infographs, tracking systems, in an elegant way. This requires NOT just graphics and NOT just writing, it needs BOTH (like 20% writing and 80% graphics). This requires a serious, dedicated and attentive to details worker.
