need a simple form created. I will provide 110 shoe images and I want my viewer to be able to select 10 shoes maximum from the choices given via check box under each shoe. They must also input their name and submit page. After the form is submitted they will receive a thank you message. In the backend I want to see a graph of the most selected shoes via rank of all the shoes. I will provide the images. I need the form created now. $25
