Hello there, We would like to develop wise HTML responsive forms for our websites as follows: Fornt-end dynamic forms: 1. Newsletter subscribe as a layer on the home page (First name, Last name and Email address); 2. Newsletter unsubscribe (Email address, Reasons of unsubscribe each by radio button) - In case of unsubscribe, email address must move to unsubscribes list in Admin automatically; 3. Sign Up (Gender, First name, Last name, Date of birth by mm-dd-yyyy select boxes, Email address as the ID, Password, Re-passwrod, Mobile phone, Country by select box, Verification code as not a robot, Checkbox subscribe for newsletters, Checkbox agree with the terms and conditions); 4. Sign In (Email address, Password); 5. Forgot Password (Email address); 6. Change Password (Password, Re-passwrod); 7. Credit cards (Visa, Master, Amex and Discovery); 7. All above filed should indicate wise errors before submit. Back-end (Admin); 1. Admin sign in (Email address and Password); 2. Users list (Date of sign up, Time of sign up, Gender, First name, Last name, Date of birth, Email address, Mobile phone, Country, IP address, Type of OS, Type of internet browser, Newsletter status, Suspend users, Deleted users); 3. Newsletter compose and save (Upload images, HTML, Type of fonts and etc); 4. Newsletter Preview and Send (Newsletters must not send to emails in unsubscribes list); 5. User's Credit cards information list; 6. Admin Sign out. I can provide you some templates as the reference. Please let me know the Type of platform, Processing times and Fees.
