I am looking for a pre-registration form similar to: [login to view URL]

However I would like to include social media integration which allows customers to sign up (and us capture customer details - Name, Mobile, Email and DoB) and automatically comes up with a share box once they complete the process.

We must be able to download the data (in order to contact them about the launch weekend) - I also want to know the % of people who actually shared after they sign up.

I have seen these before and although its not clear you don't have to share most people actually do.

I can have the graphic's for a background completed - its more the technical side and seeing if we can get the viral effect.

This needs to be a quick turn around 7 days max and I am looking for a quote for the entire project.
