Create an editable PDF from from existing PDF form

I have an existing PDF form that I would like to be editable. The form is a PDF data entry form with some simple calculations on it. Calculations are multiplications and sum. The form has something around 30 to 35 fields for editing. The goal is the user to enter data in editable columns. Some small validation like using number in number field should be considered. I have attached the sample very simple PDF that I would like to see if you can make it editable and apply some calculations on it. This is not my final pdf form but once you show me you have done this I can give you the real form. For calculations and labels on this sample form I don't care what you want to put. Again this is used just for proof of concept. If you can do this there much more PDF forms will come afterwards. The only thing that will convince me you can do this is you do the sample proof of concepts. ## Deliverables See attachment for the very simple PDF form (Proof of concept)
