Custom Powerpoint Template

I have attached a file with my cover art for my audio book. I want a powerpoint template with the audio book in the lower right corner.

1) This is form an North American audience.

2) The book deals with human psychology and helping people get "unstuck" and move forward to a successful life. It helps them with "change".

3) A footer that says -

***"Getting Unstuck - Discover What Keeps You Stuck & Learn to Sustain Positive Change'

***

also an alternate template with footer saying -

***"Getting Unstuck - Learn What Holds You Back So You Can Move Forward"***

I want the finished powerpoint template and the psd file unflattened - so i can access and manipulate the file later if need be.

You are the artist, I am not. So just make a couple (so I can decide on the look) nice looking templates keeping in mind that I populate the page with writing and pictures.

I use Powerpoint 2007 on my Windows 7 PC.
