Desktop application. Database of customers.
Must be able to identify businesses in selected radius.
User will enter business information into database. Name, address, phone, claim summary, SIC code, claim amount, phone.
User will choose SIC codes of interest and radius of interest.
Query will show businesses that meet the selection criteria.
Report can printed out with Business, distance from selected address, phone number, claim summary, claim amount.
longitude and latitude should be database fields, but the information must be calculated from the address rather than input by the user.

1) All deliverables will be considered "work made for hire" under U.S. Copyright law. Employer will receive exclusive and complete copyrights to all work purchased. (No 3rd party components unless all copyright ramifications are explained AND AGREED TO by the employer on the site per the worker's Worker Legal Agreement).

That would be an acceptable solution. The progam will have internet access.

The data will to be inputted into the database. This will be done by the user. Lat/Long must be calculated from the information rather than entered by the user. So.. there is no specified format for the database at this point.

SIC code is a list of codes that describe the general nature of the business that is entered in the database. The program should be able to select businesses within a specified radius that match a multiple selection of SIC codes. The SIC is a 4 or 5 digit number, I believe.
