Convert Windows Batch File to a GUI exe file

I have a small Windows Batch File. I would like to make it more user friendly and would like it converted into a windows GUI type application.. Simple form which asks for user input and then runs a sql scrip to return results and from the results produce an output file 1. Enter Server Name: text field {server_name} 2. Select Folder to Save File To: Folder browser field {save_location} 3. Click a Go button which will pass {server_name} into a SQL script that returns a list of names with an associated number 4. Select the number(s) you require.{contest_name} 5. pass either one {contest_name} into SQL script to create a CSV file and save to 2 above {save_location} or ability to select multiple numbers and save to multiple csv files. 6. Option to run again from point 3 above(s) Unfortunately i cannot provide a DB for testing, but i can test myself once ready..
