Contact Me Page

I have a website that is hosted on a unix server. I am trying to create a contact me page with 10-15 questions so people can send me their info. Once a user clicks submit, I would like to get an email with all the information that was left on the page. Currently the page contains some text fields, combo boxes, and checkboxes. After clicking submit, I would like to the user to be directed to a "Thank You" page.
