Symptom checker plugin

I need to add a symptom checker function to my website www.topicalmedicine.com. The specialty is dermatology with a focus on acne, hair loss, topical pain relief, sports medicine and hormone replacement therapy. This website is a plugin and because it will be unnavigable re-scaled to include other areas of medicine I would like to keep it low cost. A simple plugin installation and customization would do. I am also looking for an individual that can help me first, project manage my Teledermatology/telemedicine platform. This would need to be fully HIPPA compliant with HD quality, and scalable with keeping low bandwidth area infrastructure in mind.
