Reasonably Simple Template Design and Logo

I need a graphic person to come up with a logo and basic template for a project I'm working on.
There's a tentative color scheme and a logo icon (have .eps file), and there is *no* coding required, a plain .psd file with the basics laid out is just fine. The design should be simple, clean and straightforward because this will be the core template that comes with the package and people will want to be able to add their own graphics and artwork. Colors are negotiable, so are fonts.
In addition to payment, you will be permanently credited as the template designer with a link to your site or resume, whatever you wish.
